Do You Need a T3 Uptake test?

Understanding your thyroid health can be confusing, especially when faced with tests like the T3 Uptake test. Despite its name, this test doesn’t measure T3 hormone levels directly but instead evaluates the proteins that transport thyroid hormones throughout your body.

What is your T3 Uptake?

This lab test is a bit confusing because it has “T3” in its name which is one of the thyroid hormones but it’s not measuring T3 hormone levels.

Just stick with me for a few moments and this will make sense…

The test is evaluating proteins that attach to your thyroid hormones and not the level of hormones themselves. These proteins are important because they help transport hormones to your cells and hormones that are attached to a protein act as a reserve supply of hormones for future use. Without protein bound hormones dramatic fluctuations of hormone levels would occur.

What a T3 Uptake Test measures?

Your thyroid hormones (T4 and T3) are in two different states:

  • Bound: attached to a protein…the most important binding protein is called TBG (thyroxine binding globulin). This protein is produced in your liver.
  • Free: not attached to a protein.

Your total T4 and total T3 test results combine the bound and free levels of hormones. Free T4 and T3 measure only the level of these hormones in their free state.

Thyroid hormones must be in their free state (not attached to a carrier protein) so they can enter your cells.

“Thyroxine-binding globulin (TBG) is one of three major transport proteins, which are primarily responsible for binding to and transporting thyroid hormones to the necessary tissues.”

Why It’s Important?

The T3 Uptake test is a valuable tool for assessing thyroid function beyond just hormone levels. It helps identify whether thyroid issues stem from the gland itself (production of T4 and T3) or from factors affecting hormone transport and binding…factors NOT related to the thyroid gland.

By assessing thyroid hormone transport and binding protein activity, it helps clarify whether hormonal imbalances, liver function, or medications are influencing thyroid health.

How T3 Uptake Fits In a Comprehensive Thyroid Test Panel

A comprehensive thyroid panel provides a complete picture of thyroid function, hormone balance, and underlying issues affecting metabolism, inflammation, and energy production.

TSH (Thyroid-Stimulating Hormone) – The Thyroid Regulator

  • Produced by the pituitary gland to stimulate the thyroid to produce the hormones T4 and T3.
  • Role in relation to T3 Uptake: T3 Uptake can help clarify why T4 and T3 levels may appear abnormal despite a normal or abnormal TSH.

Total T4 (Thyroxine) – The Storage Form of Thyroid Hormone

  • Measures total thyroxine (T4), including both bound (inactive) and free (active) hormones.
  • Role in relation to T3 Uptake: Since T4 is mostly bound to proteins (like TBG), T3 Uptake helps evaluate if abnormal T4 levels are due to binding protein changes rather than a true thyroid disorder. Elevated TBG can cause elevated T4 levels on testing.

Free T4 – The Active Form of T4

  • Measures unbound (free) T4, which is available for use by your cells.
  • Role in relation to T3 Uptake: T3 Uptake helps determine whether Free T4 levels are affected by TBG levels, offering insight into potential binding issues.

Total T3 (Triiodothyronine) – The Active Thyroid Hormone

  • Measures total T3, including both bound and free forms.
  • Role in relation to T3 Uptake: If T3 levels appear abnormal, the T3 Uptake test can help determine whether the cause is due to TBG variations rather than true thyroid hormone level dysfunction. Elevated TBG can result in elevated total T3 levels on testing.

Free T3 – The Most Active Form of Thyroid Hormone

  • Measures unbound (free) T3, which cells can immediately use for metabolism and energy.
  • Low Free T3 with normal T4 can indicate poor T4-to-T3 conversion (common in hypothyroidism and stress-related thyroid dysfunction).
  • Role in relation to T3 Uptake: Helps differentiate whether low Free T3 is due to binding protein imbalances or poor conversion from T4.

Reverse T3 (rT3) – The Inactive Form of T3

  • Measures Reverse T3, a biologically inactive form of T3 that increases under stress, illness, or chronic inflammation.
  • High rT3 can indicate thyroid hormone resistance or adrenal gland dysfunction.
  • Role in relation to T3 Uptake: High rT3 with altered T3 Uptake may suggest a problem with thyroid hormone transport rather than production.

Thyroid Antibodies (TPOAb & TgAb) – Markers of Autoimmune Thyroid Disease

  • TPO Antibodies (TPOAb) – High levels suggest Hashimoto’s thyroiditis (hypothyroidism) or Graves’ disease (hyperthyroidism).
  • Thyroglobulin Antibodies (TgAb) – Like TPOAb, it helps diagnose autoimmune thyroid disorders.
  • Role in relation to T3 Uptake: Autoimmune thyroid diseases can cause fluctuations in TBG levels, affecting T3 Uptake results. This occurs primarily due to the inflammation that occurs with autoimmune diseases.

Comprehensive Metabolic Panel (CMP) – Supporting Tests for Thyroid Function

  • Measures electrolytes, liver function, kidney function, and blood sugar levels, all of which can impact thyroid health.

Important tests on the CMP:

  • Glucose & HbA1c – High levels are linked to elevated blood sugar and/or insulin resistance, which can interfere with thyroid hormone conversion. Insulin resistance can cause a fatty liver which is where TBG is produced.
  • Liver enzymes (ALT, AST, ALP) – The liver converts T4 to T3, so liver dysfunction can affect T3 levels and the production of TBG.
  • Sodium & Potassium – Imbalances can indicate adrenal stress, which often coexists with thyroid dysfunction.

T3 Uptake (T3U) – Evaluating Thyroid Hormone Transport

  • Measures the availability of binding proteins (like TBG) that carry thyroid hormones in the blood.
  • High T3 Uptake suggests low TBG (common in hyperthyroidism, liver disease, or high androgen levels).
  • Low T3 Uptake suggests high TBG (common in hypothyroidism, pregnancy, or high estrogen states).
  • Role in the thyroid panel: Helps distinguish whether abnormal T4 and T3 levels are due to binding protein imbalances or actual thyroid dysfunction.

Factors Affecting T3 Uptake

  • Estrogen-containing medications such as oral contraceptives or oral hormone replacement therapy may increase thyroid-binding protein concentrations, thereby reducing T3 uptake.
  • Pregnancy increases thyroid-binding proteins, decreasing T3 Uptake levels.
  • Liver disease may affect the production of thyroid-binding proteins, thereby influencing T3 uptake.
  • Kidney disease can alter the clearance of thyroid hormones, potentially affecting T3 uptake.
  • Glucocorticoids including cortisol from the adrenal glands can decrease TBG synthesis.
  • Androgens such as testosterone or DHEA can decrease TBG synthesis. Elevated androgens are often seen with PCOS (polycystic ovary syndrome).
  • Nicotinic acid (niacin / Vitamin B3) can decrease TBG synthesis
  • Raloxifene, tamoxifen, mitotane, fluorouracil, methadone and heroin and other opiates can all increase TBG and therefore decrease T3 uptake.
  • Nonsteroidal anti-inflammatory drugs (NSAIDs) NSAIDs…like aspirin and ibuprofen can lower serum thyroid hormone concentrations, principally by interfering with the binding of T4 and T3 to serum carrier proteins.
