Leaky Gut: When the Intestinal Barrier Becomes Compromised
One of the most discussed concepts in functional medicine is intestinal permeability, more commonly referred to as “leaky gut.” Although the term may sound like a buzzword, intestinal permeability is a real physiological process that has been extensively studied in medical research.
The lining of your small intestine serves as an incredibly sophisticated barrier. Imagine it as a selectively permeable screen that carefully determines what is allowed to pass into your bloodstream. Nutrients such as amino acids, vitamins, minerals, healthy fats, and glucose are permitted to pass through after proper digestion. Larger particles, bacteria, toxins, and undigested food proteins are typically kept inside the intestinal tract, where they belong.
This barrier is held together by microscopic structures called tight junctions. These junctions regulate what moves between the cells lining the intestine.
Under certain circumstances, however, these tight junctions can become disrupted. Factors such as chronic stress, poor diet, excessive alcohol intake, certain medications (particularly frequent NSAID use), infections, chronic inflammation, dysbiosis, and some food sensitivities may contribute to increased intestinal permeability.
When this occurs, substances that normally remain inside the digestive tract may gain increased access to the immune system beneath the intestinal lining.
Your immune system recognizes many of these substances as foreign and responds by producing inflammation. Over time, this persistent immune activation may contribute to chronic inflammatory conditions and may play a role in the development or progression of autoimmune diseases in susceptible individuals.
It is important to understand that leaky gut is rarely the only problem. Instead, it is often one piece of a much larger puzzle involving digestive dysfunction, immune regulation, microbiome imbalance, nutrition, stress, and lifestyle.
In functional medicine, the goal is not simply to “treat leaky gut.” Instead, we ask a more important question:
Why did the intestinal barrier become compromised in the first place?
Finding and addressing those underlying contributors is what helps create lasting improvements.
Food Sensitivities vs. Food Allergies
Another important concept in functional medicine is understanding the difference between food allergies and food sensitivities.
Although many people use these terms interchangeably, they are very different.
Food Allergies
A true food allergy involves an immediate immune response, typically mediated by IgE antibodies. Symptoms often develop within minutes to a few hours after eating the food and may include:
- Hives
- Swelling of the lips or throat
- Difficulty breathing
- Vomiting
- Wheezing
- Anaphylaxis (a potentially life-threatening emergency)
Food allergies require careful medical evaluation and should never be ignored.
Food Sensitivities
Food sensitivities are often much more subtle.
Instead of producing an immediate reaction, symptoms may not appear for several hours—or even a day or two—after eating the triggering food.
This delayed response makes food sensitivities difficult to recognize without careful investigation.
Common symptoms may include:
- Brain fog
- Fatigue
- Headaches
- Joint pain
- Bloating
- Gas
- Diarrhea
- Constipation
- Skin problems
- Sinus congestion
- Generalized inflammation
In some individuals, repeatedly consuming foods that trigger an inflammatory response may contribute to ongoing immune activation, particularly when intestinal permeability is also present.
This does not mean everyone needs to eliminate dairy, gluten, eggs, soy, or other common foods.
Functional medicine tries to avoid unnecessary dietary restrictions. Instead, we focus on identifying foods that appear to be problematic for the individual patient through a combination of history, clinical judgment, elimination diets when appropriate, and carefully selected laboratory testing when indicated.
The goal is personalization—and to create enough change it the diet so healing can occur and food tolerance improves
The Gut Microbiome: Trillions of Tiny Partners in Health
One of the most exciting areas of modern medicine is the study of the gut microbiome.
Your digestive tract is home to trillions of microorganisms, including bacteria, fungi, and other microbes that live in a remarkably complex ecosystem.
While the word “bacteria” often carries a negative connotation, the overwhelming majority of these organisms are either beneficial or harmless. In fact, many are essential for good health.
A healthy microbiome helps:
- Digest certain foods
- Produce important vitamins
- Support immune system regulation
- Protect against harmful pathogens
- Maintain the intestinal barrier
- Produce beneficial compounds called short-chain fatty acids
- Influence metabolism
- Help regulate inflammation
When this microbial community becomes unbalanced—a condition known as dysbiosis—the effects can extend far beyond the digestive tract.
Research has associated dysbiosis with a wide range of chronic health conditions, including obesity, insulin resistance, autoimmune diseases, inflammatory bowel disorders, allergies, mood disorders, and metabolic dysfunction.
Functional medicine recognizes that restoring a healthy microbiome is often an important part of improving overall health.
This may involve improving dietary quality, increasing fiber intake when appropriate, addressing infections or bacterial overgrowth, reducing unnecessary antibiotics, supporting digestive function, and using targeted probiotics or prebiotics when clinically indicated.
Every patient is different, and there is no single probiotic or dietary strategy that works for everyone.
The Gut-Brain Axis: A Two-Way Conversation
Many people are surprised to learn that the digestive system and brain are in constant communication.
This communication network is known as the gut-brain axis.
Rather than functioning independently, the gut and brain continuously exchange information through hormones, immune signaling molecules, the nervous system, and metabolites produced by the microbiome.
This helps explain why emotional stress can produce digestive symptoms and why digestive disorders may influence mood, cognition, and overall well-being.
Have you ever experienced:
- “Butterflies” before giving a presentation?

The Vagus Nerve and the Autonomic Nervous System
One of the major communication pathways between the gut and the brain is the vagus nerve.
The vagus nerve is the longest cranial nerve in the body and serves as a major component of the parasympathetic nervous system, often referred to as the “rest and digest” system.
Its responsibilities include helping regulate:
- Digestion
- Stomach acid production
- Pancreatic enzyme secretion
- Intestinal movement
- Heart rate
- Inflammation
- Communication between the brain and digestive tract
When the body is under chronic stress, the sympathetic nervous system—the familiar “fight or flight” response—can become dominant.
In this state, digestion is often deprioritized. Blood flow is redirected away from the digestive organs, stomach acid production may decline, digestive enzyme secretion may become less efficient, and intestinal motility can become disrupted.
Over time, this may contribute to symptoms such as bloating, constipation, diarrhea, indigestion, reflux, or poor nutrient absorption.
Functional medicine recognizes that improving health isn’t simply about changing diet or taking supplements. It also involves supporting healthy nervous system regulation through adequate sleep, stress management, physical activity, mindfulness practices, breathing exercises, and other lifestyle interventions that help restore balance between the sympathetic and parasympathetic nervous systems.
Healthy Digestion Begins Before Nutrients Reach Your Cells
Many people assume that simply eating healthy foods guarantees good nutrition.
Unfortunately, that’s not always the case.
Before nutrients can nourish your cells, they must first be properly broken down and absorbed.
Digestion begins in the mouth with chewing and continues through a carefully coordinated sequence involving the stomach, pancreas, liver, gallbladder, and small intestine.
Stomach Acid
Contrary to popular belief, low stomach acid is more common than many people realize, particularly as we age or in individuals taking acid-suppressing medications for extended periods.
Adequate stomach acid helps:
- Begin protein digestion
- Absorb important minerals such as iron, calcium, magnesium, and zinc
- Release vitamin B12 from food
- Help protect against harmful bacteria entering through the digestive tract
When stomach acid is insufficient, digestion becomes less efficient and nutrient deficiencies may gradually develop despite eating a healthy diet.
Digestive Enzymes
The pancreas produces digestive enzymes that break down proteins, fats, and carbohydrates into smaller molecules that can be absorbed through the intestinal wall.
If enzyme production is impaired, food may not be completely digested, leading to symptoms such as bloating, excessive gas, abdominal discomfort, and nutrient malabsorption.
Bile
Bile, produced by the liver and stored in the gallbladder, is essential for digesting and absorbing dietary fats and the fat-soluble vitamins A, D, E, and K.
Without adequate bile flow, even a nutrient-rich diet may fail to provide the body with everything it needs.
Functional medicine pays close attention to each step of digestion because even subtle impairments can have significant downstream effects on overall health.
