Before scheduling your initial consultation, please complete this brief eligibility questionnaire. It takes less than one minute and helps ensure that I am able to provide care in accordance with Illinois telehealth regulations.
Please note: To book an Initial Consultation, you must answer “Yes” to all of the eligibility questions.

2. Age Verification *
Are you at least 18 years of age?
3. Acknowledgement *
- I will be physically located in the state of Illinois during my telehealth consultation.
- I am at least 18 years of age.
- All appointments are conducted via secure telehealth.
- Sexton Functional Medicine does not participate with insurance. Payment is due when scheduling the appointment.
- Completing this questionnaire does not establish a doctor–patient relationship. A doctor–patient relationship is established only after Dr. Sexton has reviewed my information, accepted me as a patient, and my initial consultation has begun.
